MAPS Consent Form
When your pet is prescribed certain controlled substances, Michigan law requires us to review their medication history first. This form gives us your consent to do so.

Our online forms are the fastest way to handle routine paperwork, but they are not for urgent concerns. If your pet needs to be seen quickly, please call our front desk at 517-787-5807 during our regular clinic hours rather than submitting a form.
